About This Activity

Support the Diabetes Health and Wellness Institute (DHWI) by fundraising or registering to walk or run in the 2014 Healthy Harvest Fun Walk/5K Run and Diabetes Expo to continue the DHWI's efforts to serve the community in southern Dallas!

For the first time, we have team fundraising available to help support medical care, research and wellness programming at the DHWI. Invite your family, friends or co-workers to form a team and raise funds that will change lives in southern Dallas. Team members may run or walk in the event by paying the registration fee. Those who register to walk or run may pick up a T-shirt at the event finish line. Check out the Team Fundraising website for more information and tips for getting donations.  

The Healthy Harvest Fun Walk & 5K Run raises funds and awareness for the programs of the DHWI, a collaboration between Baylor Health Care System and the City of Dallas, to provide access to quality health care and disease prevention services to southern Dallas. Residents in surrounding ZIP codes are three-times more likely to die from diabetes compared to those living in Dallas. Prior to the DHWI's opening, the area lacked services that are essential to living healthy, such as access to doctors' offices or hospitals, healthy food options and grocery stores, exercise programs, and affordable transportation. The mission of the DHWI is to improve the care of people with diabetes by creating a new care model focused on health care, education and research. It is Southern Dallas' first and only facility addressing health and wellness needs related to diabetes. Staffed by a team including a physician, nurses, care coordinators and diabetes education specialists, the DHWI offers a simple yet innovative approach to combating diabetes and has woven itself into the culture of this community to impact societal change.

Entry Requirements:


  • Only one participant per application.
  • Waiver MUST be signed in order to process application.
  • All participants MUST finish within two and a half hours from race start time.


Race management reserves the right to postpone or cancel the event due to events out of management's control such as a natural disaster or other emergencies. No refunds will be issued under these circumstances. Management may also alter the course and distances for the safety of the athletes.

Healthy Harvest Fun Walk/ 5K Run Fundraising Contest

Official Rules

  1. Contest and Sponsor: Participation in the Diabetes Health and Wellness Institute Healthy Harvest Fun Walk/ 5K Run Fundraising Contest(the “Contest”) constitutes entrant’s full and unconditional agreement to these Official Rules. The Sponsor of the Contest is Diabetes Health and Wellness Institute (the “Sponsor”), who will be conducting the Contest and whose principal place of business is 4500 Spring Ave., Dallas, Texas 75210.  All decisions related to, as well as all interpretations of, these Official Rules by Sponsor shall be final and binding. The Contest is subject to all applicable federal, state and local laws and regulations and shall be void where prohibited or restricted by law.
  2. Contest End Date:  The Contest ends at 12:00 pm (Central Time) on October 24, 2014. 
  3. Eligibility: The Contest is open to U.S. residents who are at least eighteen (18) years of age as of the date of entry and who agree to comply with and be bound by these Official Rules.  Employees of Sponsor and its affiliate companies as well as the immediate family and household members of each such employee are eligible to participate in or win the Contest. By entering the Contest, you represent that you meet all eligibility criteria and agree to be bound by these Official Rules.
  4. How to Enter: To enter, simply register on the Team Fundraising Website for the Healthy Harvest Fun Walk/ 5K Run at www.active.com (the “Website”) and solicit and obtain donations through the Website. Only donations obtained through the Website will be counted to determine the winner.  Entrants may be required to provide information to Sponsor personnel for identification and verification including, for example, their name, a government issued identification, address, telephone number and email address. 
  5. Winner Determination and Verification: The winner will be determined from among all entrants and announced at the Healthy Harvest Fun Walk/ 5K Run (the “Event”). The winner will be the individual who obtains through the Website the most donations based on dollar amount as of the Contest End Date as determined from the records of the Website operator. There may be more than one winner if more than one individual obtains the most donations in the same dollar amount.  If there are multiple winners, each winner will be awarded a Prize (as defined below).  The winner does not need to be present at, or participate in, the Event to win.  All winners are subject to eligibility verification by Sponsor. An entrant is not a winner of any Prize, even if notified of being a winner, unless and until the entrant's eligibility has been verified.  Winners must continue to comply with all terms and conditions of these Official Rules, and eligibility is contingent upon fulfilling all requirements hereunder. Each winner will be notified by any reasonable means selected by Sponsor. In the event that a winner is disqualified for any reason, the entrant obtaining the next highest dollar amount of donations will be deemed the winner if such entrant fulfills all requirements hereunder.   
  6. Prize: The Contest winner shall receive an Apple iPad 4, 16 GB valued at approximately $394.86 or similar product (the “Prize”).  The winner shall bear all risk of loss or damage to the Prize after delivery to the winner.  The depiction of the Prize in any advertising or promotional materials or physical display may not reflect the actual Prize delivered to the winner.  Any Prize details not specified above will be determined by Sponsor at its sole discretion. The Prize is not transferable and must be accepted as awarded. No cash or other substitution may be made. The winner shall be responsible for any and all taxes, fees or other costs associated with winning the Prize under the laws of the State of Texas and federal income tax laws.

7.         Grant of Rights in Likeness: Acceptance of a Prize constitutes permission for Sponsor and/or its designees to use the winner’s name, address, photograph, testimonial or other likeness and/or Prize information or personal exposition (and/or any edited portion thereof) for promotional, advertising and/or publicity purposes in any media, now or hereafter known, throughout the world in perpetuity, without compensation or notice to, or further consent of, to the winner to the extent permitted by law.

8.         Suspension: Sponsor reserves the right to cancel, suspend and/or modify the Contest, or any part of it, if any fraud, technical failure or other factor beyond Sponsor’s reasonable control impairs Sponsor’s ability to properly conduct the Contest, as determined by Sponsor in its sole discretion. Sponsor reserves the right in its sole discretion to disqualify any individual it finds to be in violation of these Official Rules. Sponsor’s failure to enforce any term contained in these Official Rules shall not constitute a waiver of that provision.

9.         Disputes: Except where prohibited, entrant agrees that: (1) any and all disputes, claims and causes of action arising out of or connected with this Contest or any Prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate federal or Texas State Court located in Dallas County, Texas; and (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, and shall not include attorneys’ fees. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Contest, shall be governed by, and construed in accordance with, the laws of the State of Texas, without giving effect to any choice of law or conflict of law rules which would cause the application of the laws of any jurisdiction other than the State of Texas.
